Enrollment Form

The Happy Kidz Family Child Care requires the parents to fill up the enrollment forms and submit signed copies of all the following forms for our records in order to comply with the Child Care Licensing of the State of California requirements in an effort to ensure and provide a safe and healthy environment for children:

• Personal Rights (Form LIC 613A)
• Child’s Preadmission Health History – Parent’s Report (Form LIC 702)
• California School Immunization Record (Form PM 286)
• Parent Notification (Form LIC 9150)
